HOW DOES FUE WORK? A Treatment Timeline

FUE is a multi-step hair transplantation process that involves preparation, extraction, creation of the receptor sites, and implantation. A single hair transplant session typically takes several hours.

Planning

During a consultation at one of our hair loss clinics in Greater New York City or Boston, Dr. Robert J. Dorin will examine your scalp and inquire about your goals in order to best determine the design of your hairline.

Preparation

Dr. Dorin will begin the hair transplant process by trimming the hair in the donor area.

Extraction of Hair Follicles

FOLLICULAR UNIT EXTRACTION: A DEEPER DIVE A Step-By-Step Guide Through The FUE Procedure

1. Hair Transplant Preparation

When preparing for Follicular Unit Extraction, larger areas of the donor area must be shaved to about two to three millimeters. This allows the harvesting of the optimal number of follicular units. However, when we are doing smaller FUE treatments, extensive shaving is not necessary. FUE grafts can even be harvested without cutting the hair at all, or by shaving very narrow bands easily covered by adjacent hair. This "Zone Shave FUE" can not be accomplished when using a robot.

2. Hair Follicle Extractions

The limited invasiveness of this procedure affects only the most superficial layer of the skin. As a result, the extraction sites are microscopic. 

During FUE, hair follicles are extracted directly from the scalp in dihydrotestosterone (DHT) resistant donor areas, which are not prone to male pattern baldness. Under very high-powered magnification, individual hair follicles are isolated and removed using 0.75 to 1.00 mm punches surrounding the follicular unit. The punch enables the doctor to carefully harvest viable grafts, allowing them to be separated from the surrounding tissue without severing the underlying unit. Dr. Dorin performs this treatment under local anesthesia to ensure your comfort throughout the hair transplant procedure.

3. Creating Receptor Sites

In order for our hair transplant technicians to place the follicular unit grafts, Dr. Robert J. Dorin will create receptor sites using tiny incisions. The creation of receptor sites is the artistic portion of the procedure. Your doctor will then use our custom cut-to-size microblades to create the sites. These blades range from .7 mm to 1.3 mm in width and are uniquely created for each patient based on specific individual criteria. The size of the patient's grafts, the characteristics of the recipient skin, and the density of the graft placement determine the customization of the receptor blades.

Our innovative incision process allows the doctor to perform an ultra-refined transplant at densities of up to 50 grafts per square centimeter. Dr. Dorin plans this phase of treatment with great precision in order to produce a personalized and natural-looking appearance for each patient. Design nuances such as direction and angle of hair growth, hairline shape, crown whorl, and overall hair flow are attained by subtly varying the combination and orientation of the tiny incisions. A denser, more natural-looking result can be predictably achieved using our methodology.

4. Graft Placement

Once the doctor has completed his design, the grafts will be placed into the incisions. The grafts are carefully placed using very fine forceps. Since the placement process usually takes a few hours, we provide patients with TV for their entertainment.

The follicular units are preserved under ideal conditions during the placement process. Our technicians handle the grafts with the accuracy, care, and precision necessary to ensure optimal survival. Once the grafts have been placed, the doctor will inspect the transplanted area to ensure that the hair follicles are properly placed.

Next, a nurse or technician will use a hair dryer to blow cool air on the grafts, which will help form a natural adhesion as they are set in place. Bandages are not necessary, although we suggest you wear a baseball cap on the trip home.

AFTER YOUR FUE What Should You Expect? A Recovery Timeline

For most FUE patients, post-operative discomfort is low compared to FUT (Follicular Unit Transplantation). One of the most impressive advantages of FUE is rapid healing. We have found that after a week, the majority of patients show no signs of surgery in the donor area. With this procedure it is possible to perform follicular unit extraction treatments on consecutive days. This is often the best option for an extensively bald patient who needs a large number of grafts. With smaller treatments, in which the donor areas can be concealed, Dr. Dorin typically recommends waiting one to six-months between treatments, dependent on graft healing and new hair growth.

After FUE Hair Transplant Surgery...

The Next Day

Typically, patients come back to our clinic for a post-operative cleaning. You can gently clean your scalp, on your own, 24-hours after the procedure.

The First Week

You can return to work in as little as one to two days following treatment, but you should avoid strenuous activity. It is important for the first few nights to sleep with your head elevated. Avoid alcohol for three-days and smoking for two-weeks. During this time, you will notice some scabbing and moderate redness around the donor areas and receptor sites, which will slowly dissipate after the first one to two-weeks.

Two-Weeks to One-Month

You can resume normal shampooing, hair care, and regular activities because the grafts are now secure. We recommend you wear a hat in strong sunlight. You should use sunscreen of 30 SPF or higher on your scalp for the first two weeks. Over the course of the next couple of weeks, the scalp may also appear pink, similar to the appearance of a mild to moderate sunburn.

Two to Three-Months

You will begin to notice new hair sprouting from the scalp. However, it typically does not grow in evenly. New hair will be fine and thin but should become thicker over time. During this period, patients may notice minor shedding, which is normal and should not be a cause for concern.

Five to Six-Months

At this point, hair should be about two to three inches long and become easier to groom.

One-Year

Patients should now see the final results of their hair restoration procedure with fully grown hair that is thicker, fuller, and offers significantly more coverage.

While we believe that this hair transplant process is the best hair restoration option for most patients, there are some disadvantages to this method, which include:

A Higher Cost

Compared to Follicular Unit Transplantation (FUT), FUE can be more expensive because it is more time- and labor-intensive. Rather than removing the follicular units from a strip of scalp that has been excised, Dr. Dorin removes them individually from the donor area. This means that the surgery itself is less invasive, but is a more precise and meticulous procedure, which often results in a higher cost to the patient.

A Longer Process

Because fewer follicular units can be harvested in the same amount of time it takes to complete the FUT session, FUE treatment can also require multiple sessions to achieve desirable results. This can also lead to a longer total recovery time.

Potentially Weaker Grafts

When the follicular units are harvested from a strip of excised scalp, as in FUT, there is more tissue surrounding the grafts, protecting them from damage. However, when the hair grafts are taken directly from the scalp during FUE, they can become damaged, which renders them unsuitable for transplantation.
